Medical Withdrawal Request
If students need to drop or withdraw from courses due to a physical or mental health concern, they should follow the procedure outlined on the .
The form and documentation are reviewed by the Medical Withdrawal Committee.
The student’s financial aid record is reviewed, and if necessary, a representative from the Office of Financial Support and Scholarships, to determine the ramifications of the schedule change. Students can opt for refund or withdraw from the course (receive W grades).
Items taken into consideration are:
- If current or future financial aid status will be negatively affected
- If the student received a refund from a loan and will owe that amount of money back immediately
- If the injury/illness/death occurs near the end of the term
Notification is sent to the student, Cashiering/Student Accounts, Military and Veteran Services, and the Office of Financial Support and Scholarships, if applicable.
Return Post-Medical Withdrawal Request
Students granted a medical withdrawal must request to return to the university and provide a statement explaining their re-enrollment plan. They must also submit competent and credible documentation from a healthcare provider that they are ready to return. The healthcare provider issuing the supporting documentation must be a licensed provider who has provided treatment or care. The university has a that should be used by healthcare providers to supply helpful information. The university may require students to meet with a staff member in the Office of Student Life and Wellbeing to discuss their return or be evaluated by a healthcare provider chosen by the university.
