University of Nebraska at 51社区 MavCARD Services Cardholder Agreement
Definitions
This agreement governs the use of the University of Nebraska at 51社区’s official identification card, known as the MavCARD. The terms “cardholder” and “cardholders” refer to individuals whose names and images appear on a MavCARD. The terms “University” and “we” refer to the University of Nebraska at 51社区. The term “card” refers to the MavCARD itself.
Eligible cardholders include students, faculty, staff, and affiliates. A student is defined as any individual officially enrolled in courses at the University, whether full-time, part-time, undergraduate, graduate, or non-degree seeking, including those residing in university-operated housing facilities. Faculty and staff include all individuals employed by the University in full-time, part-time, or temporary positions, including academic faculty, administrative personnel, and support staff. Affiliates include contractors, visiting scholars, emeritus faculty, sponsored guests, volunteers, and others with an official relationship with the University who are authorized to receive a MavCARD.
Use and Ownership of the Card
The MavCARD is the property of the University of Nebraska at 51社区 and serves as an official university identification card. It is issued to eligible individuals for the duration of their association with the University and must be protected and maintained in good condition. Cards are non-transferable and are only to be used by the individual to whom they are issued. Cardholders are expected to present their MavCARD upon request while on university property.
Cardholders may not alter the card in any way, including attaching stickers, punching holes, or otherwise damaging it. A non-refundable replacement fee will be assessed for cards that are lost, stolen, or damaged. The University reserves the right to cancel or revoke a card at any time and may require the return of the card to MavCARD Services.
Card Expiration and Renewal
Cards issued to students, faculty, and staff remain valid as long as the individual maintains active enrollment or employment with the University. Cards issued to affiliates may include an expiration date based on the duration of their appointment with the University. Renewal of the card is subject to continued eligibility and the approval of university officials.
Lost or Stolen MavCARDs
Cardholders are required to report lost or stolen MavCARDs immediately by visiting the MavCARD Services Office in person, calling 402.554.2220, or using the MavCARD GET Mobile Application. Once reported, the card will be deactivated to prevent unauthorized use. If the original card is found before a replacement is issued, the cardholder must reactivate their card in person at the MavCARD Services Office and present valid photo identification (See Acceptable Forms of Identification) to verify their identity. Once a replacement card is issued, the original card is permanently deactivated and cannot be reactivated, even if found later.
The University is not responsible for unauthorized use or charges on a lost or stolen MavCARD. Prompt reporting minimizes the risk of misuse.
Replacement MavCARDs
Cardholders looking to replace a lost, stolen, or damaged MavCARD must visit the MavCARD Services Office in person and present valid photo identification (See Acceptable Forms of Identification) to verify their identity. All replacement cards are issued on site and are not mailed to cardholders.
A non-refundable replacement fee of $15.00 will be assessed for cards that are lost, stolen, or damaged. Payment may be made at the MavCARD Services desk, or for students, applied to the student’s account. The replacement fee will not be refunded if the original card is later found after a replacement has been issued.
